Jeanice Mitchell

Jeanice is a licensed physical therapist, president of Integrity Rehab and administrator of Integrity Home Health. She graduated from Loma Linda University with her Bachelor of Life Sciences and Master of Physical Therapy degrees. Practicing since 1995, Jeanice has worked in a variety of settings, including inpatient and outpatient orthopedic/neurologic rehab, acute rehab facilities, skilled nursing facilities, home health, long-term care and sports-medicine clinics. Jeanice's area of expertise is pelvic-floor rehab for men, women and children. She is a Board Certified Women's Health Clinical Specialist and has received biofeedback certification for pelvic muscle dysfunction (PMDB) via Biofeedback Certification Institute of America. Jeanice has a Certificate of Skills Completion in PF1, PF2A, PF2B and PF3 from the Herman and Wallace Pelvic Rehabilitation Institute. She is working on earning her Pelvic Therapy Practitioner Certification via Herman and Wallace Pelvic Rehab Institute. Jeanice is a member of the American Physical Therapy Association (APTA), American Urogynecological Society, Endometriosis Association, International Pelvic Pain Society, Interstitial Cystitis Association, National Association for Continence, National Vulvodynia Association, The Women's Sexual Health Foundation and APTA's Women's Health Section.

Kenneth Hooten

Kenneth is a Central Texas native and attended Lampasas High School before leaving for Texas Tech Health Sciences Center. There he earned a Bachelor of Science degree in Health Sciences and a Master of Occupational Therapy degree. Kenneth is a member of American Society of Hand Therapy (ASHT) and is a Certified Hand Therapist. Practicing since 2005, Kenneth has worked in acute care, skilled nursing, home health, inpatient, and outpatient- hospital based and private practice settings. He is an expert in orthopedic rehab of the upper extremity, adult neurological rehab, and industrial rehab. Kenneth has had the privilege to practice with multiple quality hand surgeons across the state. Outside of work, Kenneth likes hunting, fishing, kayaking, the stock market, Texas Tech Athletics, and spending time with his wife and son. He obtained his Master’s in Business Administration degree with an emphasis in Healthcare Administration from West Texas A&M in 2012.

Earline has a bachelor of science degree in nursing and a master’s degree in public health from Loma Linda University. She then went on to earn a doctorate degree in nursing from The University of Texas at Austin. Practicing as a nurse since 1960, Earline has worked in a variety of settings, including hospitals, clinics, home health and schools. She has also taught nursing at colleges and universities. Earline is particularly interested in and focused on community nursing and pediatrics. She is also certified in health services, is a public-health nurse and a member of the Sigma Theta Tau international nursing honor society. Earline’s pastimes include camping, walking, gardening and doll-collecting.

Reginald Van Velzen

Reginald (Reg) is originally from St. Catharines, Ontario, Canada, but is now at home in Texas. Reg graduated from Brock University in St. Catharines, Ontario, Canada with his Bachelor of Kinesiology (Honors) degree. He went on to earn his Doctor of Physical Therapy degree from D'Youville College in Buffalo, New York. Licensed and practicing since 2011, Reg has worked in orthopaedics, sports rehabilitation, stroke rehab, and sub acute care. Reg is a Board Certified Orthopedic Clinical Specialist, obtained a level 1 Dry Needling certification from Myopain Seminars, and a member of the American Physical Therapy Association. Reg is also certified through the Titleist Performance Institute as a golf medical rehabilitation specialist. He is especially interested in sports rehab and orthopaedics. When Reg is not working, he enjoys golfing, running, and travelling.

Kelli has been proud to be Clinic Director of Integrity Rehab in Copperas Cove since its inception in 2015. She has been with the Integrity Rehab family for close to a whole decade and happy to call it her home.  She enjoys working with a dynamic population and specializes in treatment for chronic pelvic pain and musculoskeletal disorders encompassing the total body. She attended the University of Texas receiving her Liberal Arts Degree in English in 2002. She then returned back to her hometown of Houston to complete her Master of Science in Physical Therapy at Texas Woman's University in 2007 and completed her Doctorate in 2010 at the same program. In 2012, she became an Orthopedic Certified Specialist (OCS) and completed her Manual Therapy Program at the Manual Therapy Institute to become a Fellow of the American Academy of Orthopaedic Manual Physical Therapists (FAAOMPT). She is a mentor for the Institute for Athletic Regeneration, a sports and orthopedic manual physical therapy fellowship program that works closely to have the most up to date treatments for high level sports injuries. Kelli is the co-owner of Alcove Education that provides continuing education courses to physical therapists and medical practitioners pertaining to pelvic floor dysfunctions with an orthopedic correlation around the country. She is a member of the American Physical Therapy Association's (APTA) Section of Women's Health (SOWH) and Orthopedic Section, International Pelvic Pain Society (IPPS), International Society for the Study of Women's Sexual Health (ISSWSH) and  American Academy of Orthopedic Manual Physical Therapists. Kelli is the author of the blog called Pain and Simple Blog.  She will be addressing lifehacks that can be applied throughout the day to reduce chronic pain throughout the body.  And she is the star of the Video about Pelvic Pain and Correcting Pelvic Alignment. She looks forward to working hard with her patients to get them back to their full potential!

T.J. graduated from the University of Delhi, India with a bachelor of science degree in physiotherapy. He continued his education at Texas A&M University earning his Master of Science degree in neuroscience. T.J. received his Doctor of Physical Therapy from Arizona School of Health Sciences. He is also Board Certified Orthopedic Clinical Specialist, Board Certified Sports Clinical Specialist, and a Red Cross certified Emergency Medical Responder. T.J. is a Fellow of the American Academy of Orthopedic Manual Physical Therapists. He has also completed a 6-month diploma from Harvard Business School in business analytics, economics for managers, and financial accounting. Practicing physical therapy since 2006, T.J. has experience in outpatient clinics, skilled nursing facilities, inpatient rehab and schools. When T.J. is not working, he enjoys trivia, playing tennis and long distance running. T.J. also is an ardent tennis and cricket fan.

Tyna graduated from Del-Mar College in Corpus Christi with an associate’s degree in applied science, an associate’s degree in surgical technology and a associate’s degree in nursing. She has worked in operating-room, long-term care, assisted-living and home-health settings. She has previously been the owner/operator of a nursing home and assisted-living facility.

Anne received her Bachelor of Science degree in Kinesiology from University of Texas and Master of Science in Physical Therapy from Texas State University San Marcos. She has been practicing as a physical therapist in outpatient rehab clinics since 2008. Anne’s expertise in treatment of pelvic floor disorders and dysfunctions. She has completed numerous courses from Herman & Wallace Pelvic Rehabilitation Institute. During Anne’s free time, she enjoys spending time with her family, traveling and outdoor activities.

Emily graduated in 2009 with a doctorate of physical therapy from the University of Texas Health Science Center in San Antonio. She has experience involving acute care, skilled nursing and orthopedic therapy. As a practicing therapist, she has a special interest in manual therapy and orthopedic/sports rehab. Outside of work, she enjoys hobbies such as running, traveling, making jewelry and playing with her dog, Sophie

Atul earned his Doctorate in Physical Therapy from A.T. Still University and received his Master's in Motor Neuroscience from Texas A & M University. Atul is a FAAOMPT and member of the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT). He is a Certified Orthopaedic Manual Physical Therapist from The Manual Therapy Institute as well as a Board Certified Orthopedic Clinical Specialist. Atul is a Dry Needling practitioner with a Certification in Dry Needling (DN-1 & DN-2) from Spinal Manipulation Institute. He is also a member of the American Physical Therapy Association. Atul is especially interested in spinal manipulation, manual therapy, sports rehab, movement system disorders, and neurophysiology. He has published research in motor learning and neurophysiology. Outside of work, Atul likes playing volleyball, cricket and exploring the outdoors.

Tiffany earned an Associate’s degree in Applied Science from McLennan Community College and graduate Magna Cum Laude Honors with her Bachelor of Health Science degree from Kaplan University.  She has been practicing as a physical therapist assistant since 2010 and has experience working in acute care settings. Tiffany was recently accepted into the  Doctorate of Physical Therapy bridge program at The University of Texas Medical Branch in Galveston, TX, which she will attend starting Summer 2017. Tiffany’s specialty area of interest is sports medicine and rehabing wounded soldiers.

Erika graduated from University of Miami with a Bachelor of Arts in History and Modern Languages. She went on to earn a Master of Arts in Communication Sciences and Disorders from University of Texas at Austin. Erika has experience working in elementary schools, acute care, as well as outpatient clinics. She is especially interested in bilingualism, language disorders, fluency disorders, and word retrieval. Erika has a Bilingual/Multicultural certification, and is a member of the American Speech-Language-Hearing Association (ASHA). In her free time, she likes reading, swimming, and playing sudoku.

Lance received his Doctor of Physical Therapy degree from University of Vermont. He has a Certification in Dry Needling (DN-1) from Spinal Manipulation Institute. Lance has almost completed Mechanical Diagnosis and Therapy certification with The McKenzie Institute. He has advanced training in ACL rehab and clinical experience in acute care, skilled nursing facility, and outpatient care. Lance is interested in manual therapy. He is a member of the American Physical Therapy Association. During Lance's free time, he likes to play golf, water ski, hike and play basketball.

Cecily graduated from Hinds Community College with an Associate’s degree in Applied Science of Physical Therapy. Since 2007, she has been working as a physical therapist assistant in outpatient and inpatient settings providing orthopedic rehab. Cecily is certified in CPR and first aid. Outside of work, Cecily enjoys church activities, spending time with her family, and reading. She is motivated by her favorite quote from Philippians 3:14.

April received her Certified Nurse Aide training (1996) and Vocational Nursing degree (2001) from Central Texas College. April’s passion and specialty area is in treating geriatrics. Besides working at Integrity Home Health, she has previous experience working in clinics and nursing homes. April loves animals and helping others, and in her free time enjoys spending time with her family.

Leah earned a Bachelor's of Science with an emphasis in Health Fitness from the University of South Carolina and went on to earn a Masters of Occupational Therapy from Texas Tech University Health Sciences Center. She has experience working with outpatient hand therapy and also special interest in treating overhead athletes. She is a member of the Texas Occupational Therapy Association. Leah enjoys spending time with her family, going to sporting events, and is very involved in her church. She loves travelling and being near the water in the summertime.

Gabriel graduated from University of Mary Hardin-Baylor with a Bachelor of Science degree in Exercise Sport Science, and then went on to earn his Doctor of Physical Therapy degree from Texas State University.  Practicing since 2018, Gabriel has previously completed internships in inpatient rehab and outpatient orthopedic/sports rehab.  He is very interested in manual therapy, orthopedics in the adult and athletic population, and movement impairment syndromes. Gabriel is very passionate about physical therapy and helping others reach their rehab goals. He is member of American Physical Therapy Association (APTA) and Texas Physical Therapy Association (TPTA).  In addition to English, Gabriel also speaks Spanish and Portuguese.  Gabriel previously played men’s soccer at UMHB and outside of work enjoys playing soccer, spending time with family and friends, dancing and continuing to learn.

Stephen received his Doctorate of Physical Therapy degree from Baylor Army Physical Therapy Doctoral Program.  Practicing since 2007, Stephen has experience working in multiple U.S. Army hospitals around the world.  He is a Board Certified Orthopedic Clinical Specialist. Stephen is especially interested in manual therapy and dry needling.  When he’s not working, Stephen is a Church of Christ preacher and enjoys raising chickens, goats, and horses.

Stephanie graduated from University of Texas at Arlington with a Bachelor of Science degree in Exercise Science, and then went on to earn her Doctor of Physical Therapy degree from University of St. Augustine-Austin. She became licensed in Texas in 2015, previously completing internships in acute care, outpatient orthopedic rehab, and inpatient neurological rehab. Stephanie is a Board Certified Orthopaedic Clinical Specialist (OCS). She has also completed Pelvic Floor Rehab training as well as level 1 dry needling course through the International Academy of Orthopedic Medicine (IAOM). Stephanie is member of American Physical Therapy Association (APTA), and is very interested in Manual therapy and Sports Rehab. She previously spent time in Guatemala City, Guatemala providing physical therapy services, wound care and education to the impoverished community. Stephanie looks forward to taking similar trips to other countries in the future. Outside of work, she enjoys watching her son participate in sporting activities, traveling, reading and live music.

Matt practiced as a US Army combat medic for five years before graduating from the US Army Practical Nurse Course in 2009. Besides working at Integrity Home Health, Matt has experience in medical-surgical nursing, intensive care, and served in Operation Iraqi Freedom. His expertise is in intensive care, interventional pain management, and wound care. Matt is additionally has an Expert Field Medical Badge, and is certified in Cardiopulmonary Resuscitation, Advanced Life Support and Pediatric Advanced Life Support. He is currently working towards his Bachelors of Science in Nursing at the University of Mary Hardin-Baylor. In his little free time, Matt enjoys barbecuing, traveling, going to car shows, and hanging out with his dog, Bailey.

Practicing since 2008, Kimberly received her master's degree in education from Tarleton State University and a bachelor of science degree from Southwestern Adventist University. She received her master’s degree in communication sciences and disorders from Texas Women’s University. Kimberly is a member of ASHA and TSHA. She has experience in school, outpatient and nursing home settings. Kimberly’s professional interests include autism, traumatic brain injury, dysphagia and language disorders. She enjoys reading and spending time with family and friends. Kimberly and her husband are the proud parents of four children.

Charley received her bachelor’s degree in liberal arts honors in 2011 at the University of Texas at Austin, Texas,  and then went on to earn her Doctorate of Physical Therapy in 2017 at Texas State University at San Marcos, Texas. Since graduating she has undertaken additional studies in pelvic health including bowel and bladder dysfunction, pelvic pain, male pelvic health, and pediatric pelvic health. In 2018 Charley also completed training to be a 200 hour certified yoga teacher from Yoga Yoga at Austin, Texas. Charley is a member of American Physical Therapy Association and the Academy of Pelvic Health Physical Therapy. In her free time Charley likes to swim outdoors, cook, and spoil her dog.

Samantha is a licensed physical therapist who specializes in pelvic health, and is also an Australian trained Osteopath. She earned her Doctorate of Physical Therapy from Texas State University in San Marcos, Texas. Samantha also earned a Masters of Health Science and Bachelor of Clinical Sciences degrees from Victoria University in Melbourne, Australia, and Bachelors of Medical Imaging from University of South Australia, located in Adelaide, Australia. She has worked in private practice, women’s and pediatric clinics. Samantha’s expertise is in treatment of pelvic floor disorders and dysfunctions in men, women and children. She received her board certification as a Women's Health Clinical Specialist from the American Board of Physical Therapy Specialties. She has received biofeedback certification for pelvic muscle dysfunction (PMDB) via Biofeedback Certification Institute of America. Samantha is a member of the American Physical Therapy Association and Australian Health Practitioner Regulation Agency.

Denise has a bachelor of science degree from Philadelphia University. She is a 2008 graduate of the Physical Therapist Assistant program at McClennan Community College. She began studying massage therapy in 1989 and graduated from Lauterstein-Conway Massage School as a Nationally Certified Massage Therapist. In her spare time she enjoys hiking and gardening.

Levi earned his Bachelor of Science in Exercise and Sport Sciences and Doctor of Physical Therapy degrees from Texas Tech University and Texas Tech University Health Sciences Center. He has worked in outpatient clinics, acute care and neuro rehab. Levis is skilled at outpatient orthopedic rehab and is interested in sports rehab as well. He is a member of the Texas Physical Therapy Association, American Physical Therapy Association and is also CPR/AED certified. Outside of work, Levi enjoys exercising, reading, and playing guitar.

Tony is a licensed physical therapist, and earned an Honours Bachelor of Kinesiology degree from McMaster University in Hamilton, Ontario Canada, and then earned his Doctor of Physical Therapy degree from D’Youville College in Buffalo, New York. He has a Certification in Dry Needling (DN-1) from Spinal Manipulation Institute. He has clinical experience in subacute, paediatric, and outpatient orthopaedic rehab settings. Tony is especially interested in manual therapy, and is working with the Manual Therapy Institute on becoming a Fellow of the American Academy of Orthopaedic Manual Physical Therapists.He has been a member of American Physical Therapy Association since 2013. Tony’s favorite hobbies are watching movies and playing different types of sports.
