And we understand your physician remains the vanguard of your care. For this reason, our care is tailored to your needs and designed as a complement to your physicians’ care.

For starters, we make the referral process easy for physicians, with a simple faxed referral. We’re also happy to contact physicians to obtain the necessary referral. And of course our experienced professionals follow your doctor’s protocols, call with any questions or concerns and are responsive to his or her inquiries. Your physician can also rely on in-depth, detailed evaluations and frequent reporting and discharge planning.
To facilitate the referral process, your physician can print and fill in the form then fax to the number listed on the form. You will then be contacted by phone to schedule your first appointment or home visit.
